This Privacy Policy provides information on how Fit Again Sports Therapy Limited and the practitioners who are responsible for your treatment may collect data from you and how that information may be used. We are committed to protecting your personal data in accordance with the General Data Protection Regulations (GDPR).

We will collect necessary data for the agreed purpose which is to assess and treat your musculoskeletal injury. This will include personal information which falls under the ‘special category’ of medical healthcare data. Details on your current or former physical or mental health will be collected as it may alter the treatments which can be administered. 

Data will be collected directly from you when you begin to use our services and enter our system for the healthcare of your injury, or if you submit a query to us via email, phone or the booking system. 

We will only contact you via phone, text message or email. This will be with regards to booking/cancelling an appointment, invoicing, rehabilitation programmes or if we need to refer you to your GP. If any medical information is to be sent via email, we shall encrypt it with a password.

By using the website (https://fitagainsportstherapy.com), you agree to the collection and use of information in accordance with this policy. We may collect information that your browser sends whenever you visit our website. This may include information such as your computer’s Internet Protocol (“IP”) address, browser type, browser version, the pages of our site that you visit, the time and date of your visit, the time spent on those pages and other statistics. This is primarily analysed via cookies. We use this information to learn more about how our website is used and to improve and administer it. In addition, we may use third party services such as Google Analytics that collect, monitor and analyse this data where permitted by law.

Your data will be used for the following purposes:

1)  providing you with healthcare and fulfilling our contract with you to provide the healthcare, or when it is necessary to protect your vital interests.

2) when queries/complaints are made, we will also use your personal information when it is necessary for us to establish, exercise or defend our legal rights, or to provide you with the appropriate healthcare or related services.

3) providing your GP with sufficient information so they can continue your diagnostic or treatment needs. This will only be performed if it is necessary for us to complete our duty of providing you with healthcare or when it is within the legitimate interest of other healthcare professionals who will be continuing your treatment.

4) anonymous analysis for browsing patterns of website use.

Data will be shared with the Sports Therapist/Physiotherapist/Health Psychologist and/or Clinic Manager who is/are directly involved in providing you with the healthcare. We will only share your personal information to third parties such as your GP or any private healthcare professionals you have asked us to contact. 

To comply with medical obligations, we will store your personal information for 7 years after treatment has ceased.

Your rights:

Under the GDPR, you have the right to:

– be informed for how data is to be collected and processed

– gain access (you may request a copy of the information we hold for you and we will provide it within the required timescale of 1 month)

– rectification (you can ensure we have the correct and up-to-date information on file by contacting us if details change)

– erasure (you have the right to be forgotten except if the data is needed to comply with our legal obligations or if the processing is necessary for the purposes of preventative or occupational medicine)

– restrict processing

– data portability (this is not inclusive of data we have created ourselves based on the data you have provided to us)

– object to your data being processed

– withdraw consent
